Vancouver’s Fortuna Silver Mines Inc. (TSX :FVI, NYSE :FSM) has experienced a second fatality at one of its South American mines in less than two years.

Fortuna said in a new release Monday (June 5) that a worker employed by a contractor died June 2 at the company’s Caylloma Mine in Arequipa, Peru.

The accident occurred during underground mining operations, the company said. No one else was injured, the company said.

Fortuna said in its release that government authorities have been notified and that an investigation to determine the cause of the accident is underway. Meanwhile, a “site-wide safety stop” was ordered and underground mining temporarily suspended before resuming on Sunday.

“We are focused now on ensuring the necessary support to family and colleagues and a prompt investigation into this accident involving one of our contractor employees,” said Fortuna CEO Jorge A. Ganoza.

On January 28, 2022, Fortuna reported a fatality at its Lindero Mine in Salta, Argentina. The company said at the time an investigation was underway. BIV has asked Fortuna for an update on the investigation into the January fatality.
